Sometimes this little enjoyable gadget can bring little stress to you. Yes when we lost our phone it brings a great stress to us. You lost phone have SIM which can be misused by some bad people. When this happens, what is the first thing we need to do, first you need to talk to your mobile network service provider and told them to temporarily disable your connection. After that you visit to your local police station and lodge a general diary. Here I have provided a sample lost mobile gd application letter. Make a Xerox copy of your general diary application  as it will be your general diary attested copy.

     I Manoj Shastri, son of Mohan Shastri, currently residing at Mohanpur 122/12( here mention your address) , recently lost Samsung J5 mobile phone with SIM.

     I went to Mohanpur Bazar for shopping on Friday morning around 8 o clock on 20.08.2016 and while returning from there I realized that my Samsung J5 mobile phone is lost. I informed this matter to Reliance(mention your SIM service provider’s name) customer care and told them to disable the SIM connection. Following are the details of my lost Samsung J5 mobile phone.

IMEI Number: 123456789987654 (if you don’t your phone IMEI number then write “not known”)

     So, I request you to lodge a general diary for the afore said incident and give me an attested copy of the General Diary so that I can submit this to my SIM service provider and block my SIM to prevent misuse of my SIM.
